The S.T.A.R. approach by Dr. Yves Laflamme

The S.T.A.R. approach (Superior Transverse Atraumatic Reconstruction) is a modern, minimally invasive technique for total hip arthroplasty that preserves muscles and soft tissues. Inspired by the direct superior approach, it allows for preservation of the piriformis tendon, promoting faster, safer, and more functional recovery.

“The evolution” – our preferred, less invasive approach: the S.T.A.R. approach

Technique:

The S.T.A.R. approach is a tissue-sparing method that preserves the piriformis tendon, the iliotibial tract, and the quadratus femoris muscle. By retaining a large portion of the capsule attached to the pelvis, hip stability is maintained.

The surgeon gently retracts soft tissues rather than cutting muscles. A single tendon is temporarily detached to access the joint and then repaired at the end of the procedure.

Procedure:

A small incision is made, providing access to the hip beneath the piriformis muscle, whose function is preserved in order to improve stability and reduce the risk of dislocation.

This approach promotes effective recovery and a faster return to daily activities.

Advantages:

  • Preservation of muscles and tendons
  • Less postoperative pain
  • Faster recovery
  • Better functional mobility
  • Minimal restrictions after surgery

Clinical outcomes:

Studies suggest that the S.T.A.R. approach is associated with shorter hospital stays, less bleeding, and a reduced risk of complications compared to other techniques.

Piriformis preservation

Importance of the piriformis:

  • External rotation of the hip in extension (standing position)
  • Abduction of the hip in flexion (walking)

Atraumatic anatomical approach

  • Superior transverse incision
  • Preservation of the iliotibial tract
  • Preservation of the piriformis tendon
    – Essential for stability and function
  • Preservation of two-thirds of the joint capsule
